- Application form (click on the desired format): Word format PDF format
- Two letters of recommendation (including one from teacher, school counselor or principal)
- Transcript
- Personal essay. The typed essay should include a description of hardships you have experienced and how you dealt with them, community service you have done, school or religious activities you have participated in, and your post-high school college and career goals.
Application deadline:
Completed Application Forms along with the required attachments must be postmarked or delivered no later than December 1, 2007!
